I-94 sample form/questionaire

Can anyone direct me to a website where I can look at the current version of I-94 form. I want to review the form and look at the questions that needs to be filled . I didn't see it on BCIS website.
I am thinking about traveling on AP . I still need to fill out this form right. does anyone know if I need to fill out anything else if I travel on AP.
Help is very much appreciated.
 
Nothing changed from before so far. Routine qs. your status is adjustment of status if you are using AP that is all. No tricky Qs in I94...
