I-94 related questions in the I-130?

I came here as a student (F1) in 2006, visited home in 2008 as a student and got an i94 from 2008 when i came back.
in 2009 i AOS-ed to H1 and got a new i94 as part of the approval notice. since then i never left the country.

on the i-130 question 14 asks how the relative entered the us, to which im assuming i write "STUDENT"
then it asks for the i94 record number and date arrived
what do i put there? the i94 from 2008 as student or 2009 as h1? and what is the date arrived? july 2008 or oct 1 2009 (when the h1 started)
next question is when will the i94 expire - again, which expiry the f1 i94 or the h1 i94?

this is my only confusion before filing.

The date you arrived is the date you last entered the US physically. So that would be 2008 as a student; for Q14 write the I-94 number from that entry and the date of that 2008 arrival, not the new I-94 you got from changing status (however, both I-94 numbers might actually be the same; take a close look at them).

However, you should write the date your latest I-94 will expire, for the question where it asks about when your authorized stay expires.

The numbers are the same, so this question has become really simple now.

so i thought of something else to be confused on.

I had applied for OPT EAD card and was approved. subsequently my H1 was also approved.
so what office approved my last employment authorization? does the h1 even come into the picture here as employment authorization or are they purely talking about any previous EAD cards?

oh and im applying for renewal of employment authorization - correct?

No, this is considered a first-time employment authorization for the purpose of that question. When they're talking of renewal they're referring to a scenario where you have obtained a non-OPT EAD before, and now it is about to expire or has expired.

oh, so my h1 doesnt count as employment authorization?
what about Q11. it reads
"Have you ever before applied for Emplyment Authorization from the USCIS, if yes give details"

does my opt-ead stuff go in here?
