I-94 not issued

Hi,
My wife came to USA on 28 th Jan'03 on H4 visa first time.
She filled I-94 card and the officer didn't gave her I-94 back. But officer stamped it on passport. We don't have any I-94 number in it or class of visa on the stamped one.
After going through INS website, i applied for it immediatly by submitting form I-102 today by mail.
Is it the right process.?

Can any one gone through the same situation?

Should have tried with the nearest I102 or INS office.
