I-94 Missing on H-1 Approval Notice

Hi Friends and Gurus,
I'm a first time user on this portal. I've recently being approved for H-1 with my company. This is a F-1 to H-1 simple transfer case. Currently, I'm working on OPT for my employer.
Here are my concerns:

1) I've received an approval notice for H-1, it is I-797C form. But, I've heard that usually it is I-797A, which is given as approval for H-1.
2) I-94 is missing on this approval notice. I'm extremely concerned, as I'm planning to travel to India in April. I've an old I-94 attached on my Passport, which was issued for my F-1.
3) I've to apply in Mumbai consulate for stamping of H-1. Do I need a different approval notice, may be I-797A?
4) Will I not be eligible to enter the country, when I come back from India?
5) I'm getting married on April 24th, does this effect my to be wife's H-4?

Go to the nearest Port of Entry and get a new I94. Did you accumulate any out of status period?
Did you talk to your lawyer about why you did not get I94?
Only after you get your I94 , you can continue to be considered to be on H1.
Also there is a very slim chance of POE refusing I94 forcing you to go out of the country.
You can get the H1 stamp at mumbai and can do the H4 at the same time. Plan accordingly.
 
I-797C

I-797C is the approval notice sent to the employer directly. which does not have the I-94 attached.
The approval notice which does have the I-94 attached is sent to the lawyer.
