I-94 is still in passport after leaving the US

My mother visited the USA and recently returned to the home country.

Type of Visa: B-1 visitor
Date of entry: December 2007
Original duration of stay: June 2008

Applied for extension: April 2008 (before original D/S ended)
Denial of extension: July 28, 2008
Grace period to leave the USA: 30 days
Date of departure: August 18 2008 (within the grace period)

The problem is that when she reached home, my brother found that her I-94 is still in the passport. Aren't "they" supposed to take the I-94 out when she leaves? Does she need to take any action to record that she did in fact leave the country within 30 days of the denial of extension? If so, what action is needed, so that it doesn't become a huge issue in case she visits again?

Based on the timeline given above, is this considered overstaying, and does it have any ramifications on future visits?
