I-94 for VISA revalidation

I am sending my passprt for VISA revalidation with my new H1 papers to State depertment in St. Louis

I have two I-94 with me,
one that I have received while entering US
other one is the latest I-94 received with my new H1 papers.

Do I need to send both I-94 for revalidation?

Do I need to send original copies of I-94 or just a photo copy is enough ?

----- Detach that from new H1 approval lower right side and attach the new I-94 in your passport with earlier I-94 and in cover letter write that new I-94 is stapled in passport
