I-94 Expiring in Dec 2009 for H4 child..but Visa valid until 2011

I have a question regarding extension of my daughter’s stay in USA.

I am on H1B visa status valid until 31 May 2011. Also, my wife and daughter’s visa status valid until 31 May 2011. (The problem is date on her I-94)

During our last travel outside the USA, while entering back, Immigration Officer stamped the date (Until which date we can stay in USA) on me and my spouse's I-94 is 31 May 2011.

But my daughter's I-94 got stamp of the date 08 Dec 2009 as at the time of entry she had the passport which was going to expire on 08 Dec 2009.

Now my daughter got her renewed passport which will expire on May 2014.

1. Do I need to file the petition for her Extension of Status as right now she is holding I-94 with date stamped 08 Dec 2009.
(How to apply I539 I mean which page, only for her as she is minor)
2. Or planning a trip to Canada will do it?
3. And if yes, do I need Canadian visa even if I am going for 2-3 hours?(please answer today as we are planning to go for visa for Cananda tomorrow, It may save our trip and money to the consulate )

Please guide me through the procedure for this endeavor.

----------------that is correct they give I-94 till passport validity.

--------------keep her both the passport stapled when go to Canada and enter US. US side you may have to go to border office to get I-94 and may have to pay $5 or $6.

***You can also try Deferred Inspection .you can also try going to same POE from where you entred last time in US and go to Deferred Inspection and they may correct her I-94.
