I 94 Errors

Senior members please help !!!

My visa is valid upto May 2006.My passport is expiring on May 2004. At the POE Immigration offcer told me that he can permit only upto the date which the passport is expiring. As he was not sure he went and checked with another officer and came back and printed Admitted class as H1 and an under it he wrote passport expires on may 2004. And he gave back my pasport without I 94.

As I had to catch a connection flight I didn't check it that time.Now i found out I 94 is missing. I am pretty sure that he forgot to give I 94. What I have to do now ? can i apply this in local BCIS office or have go out of country and come back..

A case of missing/lost I-94. You can file to get a replacement I-94 as long as you have proof that you were legally admitted with a valid I-94. If you have the stamp on your passport ( admitted until xx date ), either go to the local BCIS office or file form I-102 with service center to get a replacement I-94
