I-90 form question

Hi there,

does anyone know if the question "city of residence where you applied for a visa..." refers to my city of residence or to the city where the consulate is located?

Are you sending to LA lockbox address?
are you paying $185+$70?? that is to be paid if your mistake or want info changed on Card, if USCIS mistake then only $70
 
I'm planning to e-file.

INS made a small mistake on my name several years ago when my card was issued.

I've tried to correct it before but was told to hold on to the card because of an enormous backlog and because the mistake was minor.

Now I really want it corrected since it kind of bothers me.

------------------ did you used that card to enter US, does the card has same name as on passport?? did they airline give any problem taking a flight to US with wrong info on the card?

---------- just file the I-90 and if it is mistake of USCIS/INS then you only need to pay $ 70 as fees
 
I know, it's quite a pain, the website even says it can take several weeks till you get you appointment, but I guess there is no other way than to e- file?!

I used the card a lot of times to travel, never had any trouble, I had the wrong name added to my passport to avoid confusion.
