on I-765 for EAD, question 11 ask
"Have you ever before applied for employment authorization from INS?"
does OPT counts?

if "yes", it ask result and attach all documentation.
What documentation should be attached? all of the NOAs?

Any input will be appreciated!
 
OPT is indeed one form of employment authorization with some restrictions. If you are already on OPT, you need to include copies of your OPT card and receipts and needless to mention both sides of I94 to show your current legal status (F1 and then OPT). If you have not yet received your OPT, you better include just copies of your NOAs and a cover letter explaining the situation.
Keep in mind that extra documentation wont hurt you, but missing documents could cause lots of delays and problems.

However I lost my NOAs and receipts for my OPT application.
May I just explain that I lost them when I move?
 
You probably can wait to receive the card (OPT is usually processed very quickly) and then include a copy of it. If for any reason you cannot wait to receive it, then at least you need to provide the application receipt number or any thing that they can lookup your previous/pending EAD which in your case is OPT.
To be on a safe side consult with a lawyer.
