I-765 Q11

Hi all,
I have a question reagarding form I-765

I used to work with my OPT and H1 visa for almost 2 years but ended up quiting that job for good and got my B1/B2 visa afterwards. The attorney hired by my previous employer filed all the paperworks for me!

Now I'm married to a USC and trying to file I-765 ourselves.
For Question 11, have you ever applied for employment authorization from USCIS? Do I check "yes" for this question? not quite sure!

Which USCIS office? (Where do I get this info?)

Result.( if I do have to provide the documentation, what needs to be provided? copy of my opt card, copy of my H1 visa in my passport?)

Please help!!!

I'm positive that H1B does not count as employment authorization.
I'm not sure about OPT though.

Sure does

OPT and H1B count as a form of employment authorization. I think this for actually asks you to send copies of your previous EAD used for OPT. You should submit this, I think it will make the process easier since you have bene in the system longer...
