I-765 q 11- is the answer yes if i have had an EAD while one OPT.

I am applying for my GC and filing out the form I-765 for employment authorization with it.

I came to the US on Student (F1 visa) and then had a one year OPT after graduation. I had an EAD card issued which lists "Student Optional Practical Training" under terms and conditions. I have been on H-1b since and am now applying for this EAD for the GC.
Should i answer "yes" or "no" for question 11 on form I-765 (have you ever applied for employment authorization).

Yes,
I had the same situation I said yes I looked up where my University comes under USCIS offices and used that office for where I applied question. Also I used the A# on that wherever they asked for an A#

Yes, and don't forget to attach a copy of the old EAD (both front and back).
