I-508 form, please help....

Me and my wife just got back from our interview. It seemed to have gone well. The person who interviewed me gave me the I-508 form which needs to be filled out so my application can be processed. I'm a bit confused on this form, I do not know what to put in the box that says "I am employed by (Name and Address of Mission or Organization).

Also in box that says " U.S. State Department-Issued Personal Identification Number (PID)" what goes in this box? Please help, my lawyer said to make an appointment with him to have this filled out. I'm a afraid we will get charged again. I want to see if I can fill this out myself and submitted asap so there is not much delay.

I've lived in here in the US since I was 2 years old and I am now 30 years old. I currently work here in the US.

Are you in the US with an A or G visa or some other type of diplomatic status?

I came here with an E-Visa when I was 2 years old when my father worked for the Philippine Airlines. My father has been retired for about 18 years now and I am now 30 years old.
