1.I'm applying for AOS thru my USC spouse. My previous company had applied for my labor (RIR) back in 2003, but never heard back from USCIS. I have changed company now. what do I put under the last question in "Part 3 Processing information" of Form-485 ?

Have you ever before applied for permanent resident status in the U.S.?
Yes or No.
(NO is my guess :confused: )
If you checked Yes, give date and place of filing and final disposition.


2. spouse is filing I864-EZ with the most recent tax return. Are the 1st two pages of the 1040 return which show the AGI and tax portions enough or should I send all the other additional pages like schedule-a,b,d, state tax etc etc)

3. From various threads, i understand that it is not necessary to notarise 864EZ or anyother doc unless they are not in the form requested by the INS. correct ?

The answer is "No" unless you filed an I-485 before. It sounds like you didn't so, "No".

You need to include copies everything that went with the tax return, so 1040, W-2s, any 1099s etc. If you don't have this, have the IRS send you a tax transcript, which is a good alternative.

Thanks. IRS doesnt have the transcripts for Y2006 ready yet. I guess I'll have to send the copy of the return then.. thats 20+ pages :eek:
