I-485 Put on Hold

My Husband is a US Citizen and we applied for family based Green Card for me in the first week of January. The I-485 Processing is put on hold and I have got a letter from USCIS for REQUEST FOR INITIAL EVIDENCE.
The problem cited in the letter is with respect to by Birth Certificate.

I am an Indian National and I do not have the original of my Birth Certificate. I have only Photo Copy of the Birth Certificate, and I do not have my name written in the Birth Certificate. As in India, we do not name the child when we get the Birth Certificate.

So my father who is in India , got an Affidavit on the "Stamp paper" attested by a notary public. We have mentioned in the Affidavit, that the Birth certificate does not have my name and so we are preparing the affidavit. We sent this Affidavit to the USCIS. We did not send the photcopy of birth certificate.

The mail from USCIS states that, an affidavit should be accompanied by a document from the competent governmental authority confirming that certificate does not exists.
How do we prepare the document which accompanies the affidavit which is confirmed by government official.
