I-485 pending - EAD & VISA expired....what to do?

If the question is related to the dependent case only and not the primary applicant, as far as I know, the pending I-485 is enough to be in status if the dependent is not working.
If the primary applicant's EAD is expired, it is definitely required that they have an un-expired EAD to keep working, but not to maintain legal status.

Yes, if you are asking about dependent only and has not used EAD and not working now.
When you filed 485, he/she is in AOS state, so they are legal and you do not have to get extension.

Well not exactly. you still need to apply for spouse AP (if in case the need of travel arises).
