I-485 interview in three weeks. New Medical exam necessary ?

Finally, i received the interview notice.
The I-485 has been pending since Sept 2005.
I submitted the I-693 at the time of filing.
On the interview notice, it says I-693 is needed UNLESS already submitted.

Since it 's been more than 3 years, do i need a fresh one now for the interview?

No, you don't need fresh I-693. Every year USCIS states that I-693 for pending I-485 remain valid.

Thanks and I-864 questions

As Eb-1A, on I-864 form (asked for the interview), Part I: Basis for I-864, which one shall i check?
b) I filed an alien worker petition on behalf of the intending immigrant, who is related to me as my : (myself ?)
or
d) I am the only joint sponsor

Second question:
on Part 5. Sponsor's household size:
Persons NOT sponsored in this affidavit:
1) yourself

This line is automatically checked/selected indicating i can't sponsor myself?
Then would this be a conflict? The I-864 is for my self sponsorship as Eb1-A.

Any ideas ?
