I-485 form question: please help

Hello,

1. In "Part 1" of the form 485, it asks for A# (if any) --- An A# was assigned to me on my EAD card that i got when I was on F1 visa and applied for OPT( Optional practical training). Should I mention this A# in this section here or leave it blank?

2. In the same section next to (Current USCIS status) it says "Expires on (mm/dd/yyyy)":---- Would this be the date as mentioned on my I-94 attached to my H1 approval notice or the "'D/S"(Duration of stay) which was mentioned on F1 when i last enterd the country..?

3. Since i do not have a h1 visa stamp on my passport, is it a good idea to attach a copy of my h1-b approval notice with I-485 application?

You'll get a A# in your I140 approval notice if visa # is available. If not, leave it blank. It is not the A# in your EAD card for your OPT.

Since your current status is F1, put the expiry date of F1. But attach a copy of H1 approval notice

I think i understand answers for 1 and 3

However For question 2 i need clarification

Expiry date: Should it not be from I-94 attached with H1 approval notice rather than F1, because as i understand(i am not sure about this), once H1 is approved your earlier status expires.

(Back ground: I last re-entered US on F1 visa, however after that my H1 petition was approved but there is no H1B visa stamp on appsort as i did not travel outside)

Please let me know, if anybody has been in similar situation and what you have filled in your I-485 form for Expiry date
