I-485 for wife

Hi,

Our lawyer has been sleeping on our case. I'd like to find the forms and fill them out myself.

My husband's labor cert has been approved Sept 2004 and his forms are ready.

I just have to know which forms I need to submit as his wife. Aside from the forms below, do i need anything else?

Form I-485 Application to Register Permanent Residence or Adjust Status
Form G-325A Biographic Data Sheet
Form I-693 Medical Examination Sheet
Form I-131 Application for Travel Document
From I-765 Application for Employment Authorization

Also, I have applied for the above forms before from a previous application. My I140 was denied back then so this is the second time I'm filling out these forms.

My next question is, what do I put in the I485 form when asked "Have you ever before applied for permanent resident status in the US? If yes, give date and place of filing and final disposition"?

I believe you need to have him fill out an I-864 Affadavit of Support and have it notarized.

The truth. Say yes, mention the I-140 and the fact that it was denied.

Do we need to fill out an I-864 Affadavit of Support even if I have an H-1 myself? Sorry I forgot to mention that.

Also, in filling out the employment authorization form, there is a question that says "Have you ever before applied for an employment authorization? If yes, was it granted or denied?"

My employment authorization came out already even before my I140 was denied. Should I say it was granted but revoked?

say it was granted and then say the I-140 was denied.

Also u need birth certificate from the place u were born or non availability certificate and also 2 notarized statements from ur parents and relatives.

also u need marriage certificate

If you are seeking asjustment of status as a derivative, then I believe you need to fill our the 864. Your H-1 is not an indefinite status, it is a temporary, non-immigrant classification.
