I-485 Check List/Sending Addresses/Change of Address AR-11

I was wondering if anybody could give me/or point me to a checklist of things that should be included into I-485 filing.

First, I live in Michigan.
When filing all of my forms should they all be filed at one address?? Headings in the Forms and Fees sections of the forms say one thing and the forms the other. I am totally and royally confused. I heard that you can file by going to Chicago, IL and get it all done in one place.

I have graduated and now I am moving in the next ten days. Should I file my Change of Address form with the application? or send it separately? I am confused because I can still put my new address on all the forms except my medical exam since it is sealed. Please advise.

You have to send all forms to Chicago. I would say don't file change of address and just use your new address on rest of the forms. Change of address delays the process some times. Also you have to send AR-11 to some where in Missouri and not to Chicago.
