I-485 - Birth Certificate Last Name missing and Initials in parent Name

Hi,

I got birth certificate for I-485 filing, but birth certificate has couple of issues. I understand that I can get affidavit from my parents, now I need help on how to state (statement) the below issues in affidavit.

1. Incomplete full name, i.e. Last Name is NOT given. How to state this issue in affidavit?

2. The parents has Initials instead of Last Name. How to state this issue in affidavit?

3. In my wife birth certificate, Parents name have only First Names. It's same in passport also. So no discrepancies b/w Passport and BC on this. Should I get affidavit for this? if so how to state that?

Thanks for the reply. Govt.offcie did not accept adding Last Name Or Initials. IN this case, Having affidavit from parents mentioning the issue would be fine? If so, How to state that?
