I-140 documents

week back I got approval for my labor. Could any one please list down the document I nedd to submit with I-40 form. I know I should ely on whatever my attorney is asking, but I would really appreciate if any one can list down the documents needed for I-140.

Doc list

Here are the docs I was asked to submit:

__ __Completed and signed application forms, with draft copies:
• I-485
• I-765
• G-325
• G-28

Also EAD and AP application.

__ __ Copy of current and cancelled passport and all visa stamps, including original entry to US

__ A copy of your most recent I-94 card, including your last date of arrival into the US and expiration of the I-94 card.

___ __Copy of Birth Certificate (long form, indicating names of parents), and a copy of adoption certificate if applicable.

__ __Copy of I-797 Approval Notices if in the United States on H-1B or on an H-4

__ __Your SS# and a copy of the card

__ __Please provide your last address abroad before coming to the U.S.


__ __Please provide a permanent address outside the U.S.

__ 8 Photographs, per provided specifications

__ __ Medical Exam form, signed and sealed

__ ___Copy of driver’s license.



For Family Members

__ Please supply me with a list of family members who will be included in your application. Please include dates of birth and country of birth and the relationship to you.
___ __Copy of each family member’s current visa stamp and I-94 Departure Record Card

__ __Copy of each family member’s Birth Certificate (long form, indicating names of parents), and a copy of adoption certificate if applicable.

_ ___Copy of Marriage Certificate, and Divorce Decrees, if applicable.

_Complete copy of each family member’s passport, current and cancelled, include all blank pages

Copy of each family member’s EAD card (s) and I-20’s, if applicable.

Copy Spouse’s I-797 Approval Notices if in the United States on H-1B or on an H-4

Copy of each family member’s Social Security Card

Copy of each family member’s driver’s license, if applicable.

PLus, this was from the corporate lawyers who already had a lot of the info necessary, e.g., copies of H1, LCA, application docs, resume and so on.

This is exactly the information I have been looking for.

2 questions:

1) I see no mention in this list of 'Experience Letters'?

2) What Medical Exam form, signed and sealed?

I am preparing to apply for my I-140, I-485, I-765 concurrently and this type of information has been very hard to come by. Your reply would be much appreciated.
