I-134 Question

I have a very good friend that lives in the Czech Republic and I'd like to file an affidavit of support to invite him to stay here for 4 weeks this coming summer. I know what is required for the actual I-134 application itself, but I am a bit confused as to how to submit it. Once i have completed the form, had it notorized, and accumulated all of the bank statements and such, do I send in the application to the visa office in Prague, or do I send it to the person whom I am inviting? Also, what kind of forms does my friend have to file?

You send the notarised I-134 to the applicant. He/she will present it along with other documents during his/her interview. If he is applying for a B-2 visa, he will fill in DS-156 (and perhaps DS-157).
