I-134 Affadavit of Support

I am currently in the US and am a DV2007 winner with my CP interview being next month overseas. For those of you who have completed the I-134, I wanted to ask you, does the sponsor complete this form? Does he need to be related to you? If I am in the US, then I need to get a sponsor who can basically vouch for me and then get him to notarize the form right and that's it? Where do my personal bank statements then play a role? If I have the I-134, do I still need to show a certain amount from my personal bank account? I am in particular interested to hear from former CP winners who have gotten this done. Thanks in advance.

I want to know about I-134 form on which my sponsor did not mention the person of previously he sponsored or not. it is just blank and on date part,he just added his birth date.

Will it be problem or consuler officer will possibly request to refill up and noterized again.?after all with this condition notery allready issued it.

But i am still confuse whether there is any problem with it.

My sponsors annual income is 95K.

the sponsor doesn't have to be related to you, but he/she has to have sufficient income. Unless the consular officer asks for the affidavit, you don't need to volunteer it. Try to pass with your own financial evidence first.

Yes, of course, the sponsor would fill the form, and get it signed and notarized. He will need to attach all of the documents proving his income. He doesn't have to show all of the income on the form. If his salary is sufficient, that's all he has to show (doesn't have to show property, bank statements, etc.).
