I-130 Question. If husbend died, how to answer marriage and present/prior husbend Qs

Hello:

I am filling up form I-130 for my widow mother.
I will appreciate help form any one regarding how to answer following questions:

INFORMATION ABOUT RELATIVE :

8. Date and Place of present marriage(if married):
None ( Is this correct ? Or need to mention expired husbend's name here ? )
11. Name of Prior Husbend
XYZ ( Is this correct ? Or need to mention expired husbend's name in 8 and answer 11 as None ? )
12. Date mariage ended
Date when husbend died ( or leave it NULL ? )
17. List Husbend/wife and all children of your relative
Name of expired husbend and related details. ( Or answer as None and do not mention expired husbend's name here)

Thanks in advance for your help.
 
I would answer as follows:-

(8) N/A (Widow)
(11) XYZ
(12) Date when the husband passed away
(17) Mention XYZ, yourself and other immediate relatives

Would it better not to list husbands name at all and either mark the box that my mother is single. I am in the same boat and not sured what kind of documents I will have to provide to prove that my mother is a widow.
