I 130 for parents and siblings

Hi, I'm a US citizen and I have couple of questions regarding I-130.
1. My sister is married and has 2 kids. Do I have to file 4 applications (I-130) or just 1 for my sister?
2. My parents do not have birth certificate so I got the "Records not available" document. Is there a sample affidavit that I can use for missing DOB document? This affidavit will be from my Dad's elder brother and friend as my grand parents are not alive. Same for my mom.
3. Is the affidavit in number 2 above, required for filing I 130 application? I would appreciate if I could get a check list of docs to send along with my application :)
 
1.) Just one I-130 for your sister. Her spouse and children are derivatives.

2.) The contents expected in an affidavit are found on the State Department website: http://travel.state.gov/visa/fees/fees_4881.html?cid=3582 any format is OK as long as it complies with the INFORMATION requirements as described.

3.) The I-130 information is spelled out in the form instructions for the specific relationship type. USCIS uses the same website as linked above to determine the documents for each country. Each parents requires an I-130 filed by you.

IF you were planning to petition them for greencards, why were you surprised that they were refused B-2 tourist visas? It looks like the Consular Officer got it right.
