I-130 application questions pls help

I am in the process of filing I-130 for my wife in India.

I am not sure about the answers for the following questions 14 and 22.

14. If you are a lawful permanent resident alien, complete the following:
Date and place of admission for or adjustment to lawful permanent residence and class of admission
.

Is it the time and place of entry just before getting the GC? I got my GC in Philly but my trip was about a year before getting the GC on Adv Parole.


22. Complete the information below if your relative is in the United States and will apply for adjustment of status.
a) Your relative is in the United States and will apply for adjustment of status to that of a lawful permanent resident at the USCIS office in
b) If your relative is not eligible for adjustment of status, he or she will apply for a visa abroad at the American consular post in:

She is in India (Chennai), so should I put chennai,India under the second part leave the first part (a) as N/A.

since you got your GC through AOS, you would reflect your place of adjustment. That's exactly what they are asking for: Date and place of admission for or adjustment to lawful permanent residence and class of admission.
Yes, you should put Chennai - coz she is not in the US.

So should I put just the date and venue of the AOS. I got the I-551 stamp in my passport at Philly office. So should I put the date I got I-551 stamp and location as "philly". Do I need to put anything for "and class of admission".

If you are a GC holder, then fill out the date of your adjustment usually is the date on the back of your green card. If you are a naturalized citizen, then no need to enter AOS date, just choose you are a citizen and enter the date you become a citizen and location. This info is on your naturalization certificate. Haven't look at my naturalization certificate for decades, but I believe your alien # is also on the naturalization certificate.

What counts is the date your AOS was approved, not when you got the physical stamp in the passport or the physical card. If they didn't make a mistake, the "resident since" date on the card will be your AOS approval date.
 
not going to be a big problem as long as you are a GC holder..even you have put down the wrong date, they will no going to reject your filing. You can either use the date they stamp on your passport or the date on the back of your GC.
