I-130 Application - Affidavit for Parent using multiple names

Hi,

My dad is a US Citizen.
My dad is filling I-130 for me.
He uses multiple names, please advice me, what names, do I need to fill in I-130? And will I need to declare his other names used on I-130 form?

1> Dad's Marriage certificate has: XYZ
2> Dad's birth certificate has :XYZ
3> Dad's US Passport has: XYZ
4> My Indian Passport has: ABC
5> My Birth Certificate has: XYZ
6> My Marriage Certificate has: ABC

Do I need to submit any affidavit saying XYZ & ABC are same person? If so, can you please guide me with format of affidavit and who should notarize it.

Your father is listed on your own passport and marriage certificate, in addition to yourself? So somebody can look at your passport or marriage certificate and know who your father is?

Or when you say your father's name is on your marriage certificate and passport, are you just referring to last names, as in when your father's first name became your last name?

Since your dad is listed as XYZ in all his documents you will need to use that name in your application. You will also have get your passport fixed; use your birth certificate as proof that your father's name should be XYZ in your passport.

Jackolantern, all Indian passports list the name of father and mother. Marriage certificates also list at least father's names.

Not declaring, that my dad is using two names, will complicate the things, below is explained why:
I can get my passport fixed, but I will have to submit my marriage and it will still contain ABC as name.
Also I am on dependant VISA in US and it has my dad's name ABC on this VISA. And it will be difficult to change name on VISA, will it not be?
Also my husband will be filed for GC through employment and it will have ABC as my dad's name on application because of marriage certificate & my passport.

So now two questions:-
1> Is there any affidavit I can submit?
2> What if I wait for RFE from USCIS, will it delay my immigration processing?
(I am refering to our wait for 4-5 yrs just to get visa number from NVC, after our VISAs are approved by USCIS)

Note*: On All Documents in India, name is printed with father's name. below is the format:
FirstName MiddleName(Mostly father's name) LastName
e.g. Funzine Abc Ijk
