How to replace Name Change document? It is lost by Passport Agent. Thank you!

I submitted certificate of natrulization and original name change document for passport application. The passport and natrulization certificate have been returned, but the name change document is lost and passport agent in Seattle can't find it now.

How can I replace name change paper? I will need it to update other records.

USCIS doesn't have a form to replace this paperwork. Should I send a mail to USCIS local office? or make an infopass appointment?

Any good suggestions?

Did you check with the court from where you had your name changed and took oath?
They should have a copy or be able to provide you one.
