how to renew EAD

Hi, All,
I'm in Kansas City. My EAD will expire in Dec. 2002. Could someone kindly tell me how to renew my EAD? Which form I should fill out and where to download it? Which service center(NSC or Kansas City office) I should send my renew application to? Which document I should provide? How much money I should pay(money order or personal check)? The company helped me to get the first EAD. At this time, I want to do it byself.

for all the answers.
you need to fill I765 form, check (120?), 2 photos with A# written in the back, I485 receipt copy, copy of EAD (or the form in which you received the EAD).
See NSC -about us link in the ins web site for PO box address. Mention 485 receipt number and Alien number in all documents and write EAD Renewal application in block letters on the envelope.
