How to get iEAD?

What is the procedure to get iEAD?
we live in Los Angeles, which is the INS office to get it?

After 90 days from your EAD RD.

You can go to the local INS office with your ID (passport or DL),
pay check,EAD receipt and the new EAD application form to get iEAD.
 
Reciept- copy or original?

Can we get iEAD without the Pay Checks ??

Are the pay-checks absolute must for the iEAD ? Anyone got iEAD wihtout the pay-checks recently ?

Can you go and get EAD card yourself once you receive EAD receipt?

or you have to wait for 3 months necessarily?
