How to fill the SSN form

Hi all,

 I will go to local SS office for changing SSN card to remove the restriction on the card, and I just downloaded the from SS-5.

My question is that there are several choices( 1. U.S.Citizen, 2. Legal Aline Allowed To Work, 3. Legal Alien NOT Allowed To Work 4. Other ) for line 3 (CITIZENSHIP). Could you tell me which one did you choose?
