I'm filling J1 VISA WAIVER RECOMMENDATION APPLICATION Form. But I don't know how to select or fill the following items:

No.5 I am requesting a recommendation for a waiver of the 212(e) requirement based on: Which one?
(1) Exceptional Hardship
(2) Persecution
(3) Interested Government Agency (Physician)
(4) Interested Government Agency (non-physician)
(5) State Health Agency Request
(6) No Objection Statement

No.12 Purpose of the Form?

No.14 Does this application include any J-2 dependents?

#5. well, which basis are you applying on? Have you considered which one of these is right for you?

#14. How are we supposed to know whether you have any dependents on J-2?
