How to correct write an American address?

Hi everybody!

I would like to ask about correct writing of American address in position No. 7. I think that have to be as:

7а. ( с/о ): Dzerjzinky Felix
7b. (address line 1): 1010 Worner Brosers Avenue, apt. 23G, Brooklyn
7c. (address line 2): "leave empty"
7d. (City/Town): New York
7e. (District/County/Province/State): New York
7f. (Postal code/Zip code): 11115
7g. (Country): United States

Is it right?

If you go to the United States Postal Service website and click "Find a zip code", they have an application which will give you the correct address.

http://zip4.usps.com/zip4/welcome.jsp

Just type in the address as you think it should be and it will return a list of close matches if it was incorrect. It will also give you the correct zip code (which doesnt relate to your question).
 
Probably you did not seen that real address was changed by my self. All what I need to know it is technology of correct writing. The name of avenue, c/o name and zip code is fictitious. The real I will write in the form. I just need to knew how to write it in.

Strangely, I am always know that Brooklyn is NY's district but not City.

Brooklyn used to be a city, but became part of NY somewhen in the late nineteenth century. Technically, today it is called a borough; NY has five boroughs, if I recall correctly.
 
The City of Brooklyn has merged with the City of New York a long time ago, but postal addresses continue to be written as Brooklyn, NY. E.g.

1000 FLUSHING AVE
BROOKLYN NY 11206-4718

(I have no idea what's there - I just plugged in a random address.)

As nine4108 says, when dealing with an unfamiliar address, it's best to go to Google Maps to make sure it's in the right place. Then go to USPS.com to make sure you formatted it correctly.

10 Broadway
New York, NY

10 Broadway
Brooklyn, NY

10 Broadway
Staten Island, NY

are 3 different places in New York City. The first one is in Manhattan, the second one in Brooklyn, and the third one in Staten Island. I'm not even going to get into Queens (Astoria NY, Elmhurst NY, etc, etc, etc).
