How to apply for SSN for wife (was on H-4) who just got EAD ?

Can someone please list the documents required and the process to follow to get an SSN for someone who has been on H-4 and just got EAD based on husband's 485 application ?

Go to Local SSN office with the the following Documents
EAD & Passport is enough.
