how to apply for AP ?

My AP is getting expired this month. Now I want to apply for the AP extension.
- What are all the documents I need to apply for it ?
- Where do I need to send it : local INS office or CSC ?

I131, 2 photos, I485 receipt copy, lr explaining why u need AP, check.
Apply at the center where your 485 is pending.
