How to answer the question in Part 10 section D?

I am not sure what would be the right/best answer for the question in the table Part 10 D, can you please advise?:

Why were you arrested, cited, detained or charged?

I was cited 8 years ago. Citation says:

"Took possession of merchandise that was on display with the intent of depriving the merchant of the possession without paying the full retail value."

I did not have any intent to leave the store and I plead Not Guilty, but i was still found guilty during the hearing in district court because technically retailer stopped me outside the store. For this incident I was charged with fine $150 which is a Summary Offence per the statute.

The Court Disposition says:

Description of Charges - "Retail Theft - TAKE MDSE",
Grading - S (Summary Offence)
Fine - $150.

So, what should be my answer on the question - Why were you arrested, cited, detained or charged?

I thought of these options that i listed below, which one in your opinion would be most appropriate in my case? Or maybe you can recommend something better?

- Stepped out with merchandise
- Summary Offence - Take MDSE
- Take MDSE - Summary Offence
- Take MDSE
- Retail Theft - Take MDSE
- Shoplifting

Thanks for your input. Maybe better to say Petty Retail Theft?

And one more question. Do i need to enclose court disposition with the application or better to brring and present it during the interview? I am sure that my name will pop up during the FBI check because i was fingerprinted for this incident. I read instruction and it says that we must enclose court disposition, but some people suggested to bring it to the interview and not send it out. What do you think? Is it a must to enclose court disposition with application?
