How to answer some question in I-765 ?

In form I-765 - question 11, they ask if have you ever before applied for employment authorization from USCIS?
- If i used to applied for EAD card but under F-1 category, should i answer YES?

They also ask "at what date you applied?"
- What date should i use? Can i use the date in the Notice of action (Receipt date in form I-797C)? Or date i got my EAD card?

In question 10, whey ask to put A# or I-94# (if any)
- I have A# from my OPT EAD card, should i use that # in this question? Or just use the I-94#?

Your answer should be YES, even though it was based on F1.

Use i-94 no. Your EAD A# will not be your AOS A#.
 
For item #3, I guess you should be providing both I-94 # and A# since you have it. That's what I did for my 2nd AoS application as I had A# from first one

A# is assigned to an individual only once by USCIS and should be used across all applications doesn't matter if the categories/basis are different.

EAD based on OPT A# is NOT the same as AOS A#. I have had two OPT EADs and I have done AOS. Once you are assigned an A# from AOS, or EAD based on sthing else, or H visa, then that is your A# for life. But that A# that comes with EAD based on OPT is NOT you lifetime A#. I am speaking from experience. When I applied for my first OPT EAD, it came with an A#. My 2nd one 5yrs later came with the same A#. I wrote that A# on my AOS but it came back with a new A# which is now my lifetime A#.

Thank you for the answers :) I also have 1 more question that just want to make sure that i will put the correct symbol. In question 16, I need to fill out the letter and number of my category. I'm applying for AOS through marrying with US citizen, what is the symbol i should write down in the form? My understand if "C 9".. THanks again.

I have the same doubt. But I think you are right. Can someone confirm on this please. Thanks
 
c(9) is correct

Since you are filing a EAD petition as a benefit resulting from a pending I-485 petition; you should put it c(9)
 
I answered no because it was under different circumstances and it was ok. but, I don't know much about it so I would go by someone more knowledgeable :)
