How long can you stay on J-1?

Hello all,

I am offered a fellowship at a very good university hospital on J-1. The question I have is, is it possible to go on residency following the fellowship on J1 (by renewing the J1, or what-so-ever) without pursuing the home residency requirement for 2 years? Another possibility is to get J1 waiver instead of residency, my profeciency is in Thoracic Surgery. Is it possible to get this waiver in this profeciency?

Also, how hard is it go get an O-1 in case I perform well? Is it possible to get O-1 without doing home residency requirement for 2 years?

Basically, as you can see, I am trying to avoid leaving the country after finishing the fellowship on J1. And I would appreciate any help in this matter.

Yes, you can extend J-1 status to do residency after the fellowship without leaving USA.

But practically hard-as you need to show a logical progression to your training to obtain no objection certificate from home country. if you are enrolled in Gen Surgery residency to begin with and show CT surgery as your goal, that is logical. However if you finish CT surgery fellowship and then enroll in Gen Surgery residency, many questions may arise. O1 might be your best bet. Yes, you can get O1 without fulfilling J-1 waiver or returning to your home country. Get a good lawyer.
 
O is not an option

O visa can only prolong your stay here but you still need a J-1 waiver - its not an option except to prolong your six year of J1 status. You could do your residency after finishing your Fellowship without any trouble. But once you have J 1 you have to get the waiver in order to be employed here after your residency.
