Sample checklist
Everything should be prepared for each person.
Two immigration (not passport) photos for I-485; another two for I-765 (EAD Card) and another two for I-131 (Advanced Parole) if necessary. Write name with pencil on the back side of each photo;
Form I-485
Form G-325 (not required for children under 14)
Medical Examination, should be filled out by INS authorized doctors
For I-134 for each of your dependents (not required for your spouse if he/she is working with an employment verification letter)
Statement of visa status including date of entry and date of change of status, attached by all documents issued by the INS to prove your statement, including passport (no blank pages), visa, i-94, i-20, IAP-66, Practical training card, employment authority card, H-1B, H-4, F-1, F-2, J-1 Waiver, etc.
Copy of birth certificate
Evidence of marital status (marriage license, (divorce certificate - if applicable)) for each of your family members (spouse and children)
Letter of verifying your employment by the employer, indicating your title and salary consistent with those on your labor certificate application form.
Checks payable to INS (if applicable, some cases it is provided by your company)
