How far in advance should I start my H1 renewal process?

I am currently working on a H1 and the petition is valid till 06/02/2006. I would like to know how far in advance I need to start working on getting the next threes years of my H1.

Can I get the visa in the US without leaving? I did not want to risk leaving the country when my first petition got approved, but I would like to travel in the near future.

Can I get the visa in the US without leaving?
-----NO VISA stamps in US now, DOS has stoped Visa stamping IN USA
I did not want to risk leaving the country when my first petition got approved, but I would like to travel in the near future.
