Yes you need to fill out the form she sends you and sign it. It would be a good idea to attach the letter to the form (out of courtesy). The letter will not hurt, but may give more details.

Search this web site for other letter formats. There are several out there. And also search on peoples experience with Congressman's help. It has generally been very positive.

You could also go and visit the Congressman's office get to meet the case worker and get the form yourself without waiting for the mail. Personal contact does help. Just a suggestion!!
