In statistics they publish (such as http://travel.state.gov/pdf/FY05tableIII.pdf), they show seperate numbers for "China - mainland born", "Taiwan born" and "Hong Kong S.A.R." So my best guess is "seperate".

You can ask the State Dept., they do have a web form for asking this kind of question at http://contact-us.state.gov/cgi-bin/state.cfg/php/enduser/ask.php
and for a straightforward question like this, you should get a quick answer.

HK is considered as Worldwide , which is in a separated line than China!!!
