1. Form I-485 in duplicate with filing fee of $255.00, two photographs attached;
2. $50.00 service fee for fingerprinting;
3. Form I-131 with filing fee of $110.00 and two photographs attached;
4. Form I-765 with filing fee of $120.00 and two photographs attached;
5. Signature Card of formI-765;
6. Identification documents for Form I-131 and I-765;
7. Form G-325A biographic information sheet;
8. Form I-693, Medical Examination results
Supporting documentation:
Exhibition1: Evidence of valid nonimmigrant status;
a. Photocopy of Form I-797A approval notice of H-1B;
b. Photocopy of I-94 form issued with I-797A;
c. Photocopy of I-20 issued by xxx;
d. Photocopy of I-94 form issued when last entry;
e. Photocopy of all pages of passport including visa page of last entry.
Exhibition 2: Form I-797, approval notice of I-140 Immigrant Petition based on immigrant petition;
Exhibition 3: Employment letter;

another question is that I have I-485 form in duplicate to make sure the officer still

have one if another one is broken when move staples for photos (becasue I staples check and photos on the first I-485 form. Does duplicate i-485 form confuse the staff, who screen materials?
 
I don\'t think you need degree certificates for 485 anymore.

I don\'t remember sending degree certificates and experience letters for I-485.
 
Degree certificates and experience letters are not needed for I-485; also, you may want to keep your

 
 
Signature card

It\'s the little card where you provide your specimen signature and a print of your right index finger. You cannot download it from INS website as it is a card. You can get it from your local INS office or call INS or fill a web-form to have it sent to you. Takes about 2 week to get it the latter way.

because i do not know how to print fingers on it. Any advise?
