I am about to file for EAD for my wife, but I seem to have a problem. When I first came to the US almost 6 years ago my wife got EAD based on J-2 visa and this EAD has long since expired and has been lost. I am now filing for Adjustment of Status and was wondering what I should do regarding her EAD application. Any help on this matter will be very much appreciated. Thank you in advance.

Do you have the envelope in which EAD was sent? It has to part that she can mail that will have her EAD approval date and other info. She can also submit the affidavit that she has lost her EAD she need to write her name as on EAD, A# DOB, I-485 filing receipt # and need to sign with date. On cove letter mention what is attached with EAD application form and that she lost the past EAD .
