help with my h1b

Hi I am a Canadian citizen who entered America in May on a J1 Visa, I finished my J1 summer job and have started my new job which I need my H1B visa for, the visa petition has been filed by my employer through a lawyer and they have kept me out of the loop on the information. Thanks

1) I was wondering if I can change my employer once my H1B visa has been approved
2) Can I stay in America while the transfer from J1 to H1B happens.
3) Should I have started working for my employer before my H1B has been processed
4) If I choose to not stay with my original job can I quit and stay in the country while looking for a new job or will I have to leave during that time?

I am a qualified doctor and am ECFMG registered. I am applying for residency but no luck yet. Currently I am in USA on H4 status. I wanted to know if I can work at Physician's Office and if I want to then what kind of visas do I require?

I'm confused. All of the H1-B's were used up on April 1 and 2 of this year. Are you working for a non-cap exempt entity? Like a university or something? If not, then you ill have to wait until next year for an H1. In the interim, and depending on your qualifications, you might want to explore getting a TN visa as a temporary measure.
