Help with I-90!!!

Hi!
It is time to renew my Green Card. I came like refugee from Bosnia and Herzegovina. I have problem with some of the questions on the form!
Please help me if you can!
These are the questions:
1) City of residence where you applied for an immigrant visaor adjustment of status! (Icame like refugee from Croatia and I just applied in resetlament center).
2) Consulate where Immigrant visa was issued or USCIS office where status was adjusted? (I didn't get visa, I got I-94 when I arrived in New York and after I applied for green card in Memphis)
3)Date of Admission as an immigrant or adjusted status! (What date I am suppose to put here)

--------- write what was your address at that time? write that address. if you dont understand any item then write the item# and explain on cover letter
2) Consulate where Immigrant visa was issued or USCIS office where status was adjusted? (I didn't get visa, I got I-94 when I arrived in New York and after I applied for green card in Memphis)
------------- if approved at Memphis then write Memphis
3)Date of Admission as an immigrant or adjusted status! (What date I am suppose to put here)
---------- the date your GC was approved, your GC will be having that date on it.
*** FYI I dont know much about refugee cases.
