help with form I-129

Hello everybody:
I have a question regarding the form I-129 for the tn renewal
In part 2 question 3 I am supposed to enter my I-94 number. Is this number the Departure No at the top of the I-94 or the other number stamped by the POE officer that starts with TN****?. Sorry for my ignorance but this is my first renewal.
Also in part 2 , question 4 what would be the answer?
And part 2 question 5 would it be C or E?

yes, use the Departure number.
