Help on medical RFE

Hi I receive the RFE letter from BCIS.
MY RFE is for employer letter and my w2 forms.
My wife's RFE is for "tuberclonine test" (pardon my spelling here) which she missed first time because she was pregnant.

Should she take this test at the same office (We moved after that)
and can the report be in a seperate envelope.
Because the letter said that they sent back the original envelope.
Can we send two envelopes back or should it be only one as awhole.

Anybody was in this situation? Please help;
 
No, it does not have to be from the same office.
Just include a covering letter explaining why you chose a different BCIS approved doctor.
