You need to file all the forms and documents that you filed for the first H-1 and proof that you have maintained your status (paystubs). You need to file an I-129, I-129(H), I-129(W), ETA-9035 and a support letter from the employer along with a filing fee of $1,130.00. The employer MUST pay at least $1000.00 of the filing fee and INS will return the petition if you include your check. It\'s not horribly difficult but I think you will be asking for trouble to do this yourself.
